VERMONT 05354 November 7, 1972 United States Atornic Energy Corcission Ditectorate of Regulatory operations, Region I 970 Broad Street Ncuark, New Jercey 07102 REFEPJWCE: Operating License DPR-28 Docket No. 50-271 Abnorttal Occurrence No. A0-72-12 Centlement As defined by Technical Specifications for the Vermont Yankee Hucicar Power Station, Section 6.7.A.c, we are reporting the following Abnormal Occurrence ao AO-72-12 and confirming our telephone conversation vith your of fice yesterday.
At approximately 1240 on the af ternoon of November 6 with the reactor oporating at approximately 75% power, the off -Cas Sample Chanber van intentionally inolated to permit replacenent of the Sanple Chanber Flow Hetering Valve. Air ejector off-gas van unnonitored for approxinately 15 tainutes and this constituted a violation of Technical Specificationo, r Section 3.8.A.1. During the period that the Off-Cas Sanple Chanber van foolated, and aubnequent to that time, off-gas activity at the atack ohowed a conutant level of 19x103 pci/nce on the atack gas monitoring systen and a grab canplo van obtained at the air ejector.
The Sampic chattber riow Metering Valve van found to he leaking when high nirborno particulate activity in the hirbine Building initiated n nearch for leakage into the building. Besides replacement of the valve, three fittings and one union were tightened.
The off-gaa monitoring nyatem is currently operabic in accordance with Technical Specifications.
