Dear Sir:
As defined by Technical Specifications for the Vermont Yankee Nuclear Power Station, Section 6.7, we are reporting the following Abnormal Occurrence as A0-74-14 and confirning our telephone conversation with your office.
At approxinately 1115 on October 15, 1974, while pressurizing the primary centainment in preparation for the Class A leak rate test, a leak in the primary contM=nt was evident. The leak was identified as a missing 1/2 inch test connection plug on_drywell penetration X-16A, which is the Core Spray System 'A' containnent penetration.
The missing plus resulted in an abnormai degradation of one of the several boundaries designed to contain radioactive materials.
A pipe plug was immediately obtained and. installed in the test connection and pressurization of the primary containment was continued.
