ATTACHMENT TO LICENSEE EVENT REPORT 81-78/0lX-1 COMMONWEALTH EDISON COMPANY (CWE)

DRESDEN UNIT 2 ILDRS 2 DOCKET # 050-237 During normal operation while performing ADS valve operability surveillance due to HPCI being inoperable, the 2B electromatic relief valve failed to open. An immediate shutdown was begun per T.S.3.5.C. .

This event had minimal effect upon public health and safety since the remaining ADS valves and all low pressure ECCS systems were operable.

The reactor pressure was less than 90 psig within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> as required.

Failure of the electromatic relief valves has occurred before, the last being reported by R.O. 50-249/81-25.

The failure was due to a wire that became physically wedged in the contact that bypasses the hold coil. Because the contact could not close, the hold coil was placed in series with the pick-up coil thereby reducing the voltage across the pick-up coil to below the miriimum pick-up voltage and preventing solenoid operation.

The wires were fastened out of the way so as not to interfere with the contact. Inspections will be made during the next outage of -

sufficient length and during subsequent refueling outages to prevent recurrence.
