liiollThe source of the leakage was found to be pressure relief valve RV-3-8526 (Dresser 3/4" l

The relief valve, which had failed open due to external damage, will be reolaced I

and the test rerun prior to reactor startup. Any leakage in excess of the Tech. Specs.

will be reported at that time.

Local leak rate testing of these systems will continue to [

be performed during refueling outages.

,g OESCRIPTION OF EVENT While performing DTS 1600-1, Local Leak Rate Testing of Primary Contain-ment Isolation Valves, the pressure relief valve RV-3-8526 failed open causing the N2 make-up piping to fail the local leak rate test.

Leakage was found to be at a rate of 80.39 SCFH which is in excess of the Tech. Spec. 4.7.1 limit of 29.38 SCFH.
