(b), conditions leading to operation in a degraded mode permitted by a limiting condition for operation or plant shutdown required by a limiting condition for operation. This revised report includes the findings following further investigation of the cause of the event. It also revises the corrective action based on these findings and corrects the report dete. The report date 01/03/82 was incorrect. The correct report date is 01/03/83.
