SUPPLEMENTARY INFORMATION Report No.: 50-302/82-009/03L-0 Facility: Crystal River Unit 3 Report Date: March 16,1982 Occurrence Date: February 4, 1982 Identification of Occurrence:

Failure of Containment Isolation Valves MUV-259 and MUV-260 to close during cold shutdown testing. .This created an event not attributed to a specific shutdown activity as required by Regula-tory Guide 1.16, and therefore, contrary to Technical Specifica-tion 3.6.3.1.

Condit d o e Prior to Occurrence:

Mode 5 cold shutdown (0%).

Description of Occurrence:

At 0400, during performance of Surveillance Procedure SP-435, Containment Isolation Valves MUV-259 and MUV-260 failed to close. Redundancy is provided by MUV-253. Maintenance was in-itiated and operability was restored to MF/-259 and MUV-260 on Feb-ruary 5, 1982.

Designation of Apparent Cause:

The cause of MUV-259 and MUV-260 failing to close can be attributed to insufficient torque switch settings.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action:

MUV-259 and MUV-260 torque switch settings were increased and corrective action assignment 82-30 has been issued to revise MP-402, to ensure that torque switch settings are maintained above the L

minimum torque setting required to operate limitorque valves.

Failure Data:

This is the first occurrence for MUV-259,-the second occurrence tor MUV-260, and the fortieth and forty-first event reported under this Specification.
