, SUPPLEMENTARY INFORMATION l Report No.: 50-302/82-028/03L-0 Facility: Crystal River Unit 3 Report Date: April 30,1982 Occurrence Date: March 30, 1982 Identification of Occurrence:

Failure to have two (2) operable source range instruments.

contrary to Technical Specification 3.3.1.1.

Conditions Prior to Occurrence:

Mode 3 hot standby (0%).

Description of Occurrence:

At 1856, following a reactor trip, it was discovered that source range instrument (NI-2) was indicating three and one decades lower than the other instrumentation. Maintenance was initiated and operability was restored at 2335 on March 30, 1982.

Designation of Apparent Cause:

The cause of this event is attributed to a failed high voltage power supply for source range instrument (NI-2)

Analysis of Occurrence:

Redundancy was provided by source range instrument (NI-1) and the reactor was in a shutdown condition. There was no effect upon the health or safety of the general public.

Corrective Action:

The high voltage power supply was replaced and the instrument was functionally tested satisfactorily. No further corrective action is deemed necessary.

Failure Data:

This was the first occurrence of this type and this is the second report under this Specification.
