l i i o l lThe probable cause of failure was due to some dirt build-up on the valve stem. This l

[i,,j l cleared up upon cycling the valve af ter the thermal overload relays were adjusted and re- l

set; the valve tested satisfactorily. Since this occurrence, two surveillances on this  ;

DESCRIPTION OF CAUSE Unknown. Possibly thermals overloads tripped and overload reset push button out of nM untmonr.

Finally got valve to close after resetting thermal overloads and adjusting the thermal overload reset push button. Took valve out-of-service in closed position.
