Missed Surveillance: Monthly Functional Test of Steam Generator Blowdown Radiation Monitoring Channels 1Rl9, A, B, C, D.

CONDITIONS PRIOR TO OCCURRENCE:

N/A DESCRIPTION OF OCCURRENCE:

At 0945 hours0.0109 days <br />0.263 hours <br />0.00156 weeks <br />3.595725e-4 months <br /> on 6/27/78, Performance Department personnel discovered that the monthly functional test of the Steam Generator Blowdown Radiation Monitors was overdue. This test was required to be performed by 6/25/78 using the 25% tolerance allowed by Technical Specifications.

The test was immediately performed and the results were satisfactory.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was personnel error in that the responsible supervisor failed to identify the required test before it became overdue.

ANALYSIS OF OCCURRENCE:

Environmental Technical Specification 2.3.2.f states that all liquid effluent radia~ion monitors shall be calibrated at least quarterly by means of a radioactive source which has been calibrated to a NBS source. Each monitor shall also have a functional test monthly ....

Failure to perform the required surveillance is a violation of Environmental Specification 2.3.2.f.

CORRECTIVE ACTION:

Immediately upon identification of the missed surveillance, the functional test was performed. Test results were satisfactory.
