1. Report No.: 50-302/78-70/03L-0
2. Facility: Crystal River Unit #3
3. Report Date: 8 January 1979 4 Occurrence Date: 15 December 1978
5. Identification of Occurrence:

One pair of Auxiliary Building exhaust fans inoperable contrary to Technical Specification 3.7.8.1.

6. Conditions Prior to Occurrence:

Mode 1 power operation (98%).

7. Description of Occurrence:

At 0530 while performing Surveillance ?rocedure SP-335A (Radiation Monitoring Functional) a low flow condition was noticed on Auxiliary Building exhaust AHF 14 A and AHF-14 C were started and AHF-14.B and AHF-14.D secured. Subsequent investigations revealed that AHF-14D had ejected all blades. AHF-14D was repaired, tested, and returned to service 19 December 1978.

8. Designation of Apparent Cause:

The cause of this event is attributed to possible damage that occurred during event 78-064/03L-0.

9. Analysis of Occurrence:

There was no safety hazard to the plant or general public as redundant fans AHF-14A & C were operable.

10. Corrective Action:

No corrective action necessary as all fan blades were replaced.

11. Failure Data:

Seven previous occurrences, but first occurrence of this type.
