[Es IAt 1100 while performing a visual inspection of RCP-A seal package, pres- I r.u s. tsere boundary leakage was discovered from a crack in the weld between the i o4 Ithird stage cavity and a vent pipe on the seal. SUPPLEMENTARY INFORMATION REPORT NO: 30-302/82-004/01T-1 FACILITY: Crystal River Unit #3 REPORT DATE: .Atly 26,1983 OCCURRENCE DATE: January 29,1982 IDENTIFICATION OF OCCURRENCE:

Reactor coolant pressure boundary leakage was discovered on Reactor coolant pump "A" (RCP-A) seal package. Technical Specification 3.4.6.2.a requires no pressure boundary leakage.

CONDITIONS PRIOR TO OCCURRENCE:

MODE 3 (HOT STANDBY).

DESCRIPTION OF OCCURRENCE:

While performing a visual inspection of RCP-A seal package, a crack in the weld between the third stage cavity and a vent pipe on the seal was discovered. The plant was cooled down to Mode 5 from Mode 3 and repair was initiated.

DESIGNATION OF APPARENT CAUSE:

The weld crack was apparently a result of mishandling the seal during installation.

ANALYSIS OF OCCURRENCE:

There was no effect upon the health or safety of the general public.

CORRECTIVE ACTION:

i l The seal package for RCP-A was replaced with a rebuilt spare seal package. Procedures 'vhich control the installation of the seal packages have been revised to stress the importance of

! proper handling during seal ir.sta!htion.

FAILURE DATA:

This is the first occurrence concerning RCP-A and this is the third event reported under this Specification.
