Auxiliary Building Exhaust Monitor, reported in accordance with Environmental Techn4<=1 Specification 2.4.2.G.

Conditions Prior to Occurrence: Mode 6 refueling j 1

Description of Occurrence: At 0545, Auxiliary Building Exhaust Monitor, l RM-A2 (gas), interlocks failed to respond to a high radiation alarm during the performance of SP-346, containment penetrations weekly check during refueling operations. Investigation by control techni-cians revealed a failed surge capacitor across K1022 relay. Grab samples were taken and analyzed on a daily basis beginning at 0600 )

on 3 June 1978. Operability was restored 6 June 1978.

Designation of Apparent Cause: Failure of surge capacitor across relay K1022 thus preventing interlock function. l Analysis of Occurrence: There is no hazard to the health and safety of -

the plant or public as daily grab samples were taken and analyzed for -

gross radioactivity. No detectable gas discharge.

Corrective Action: No corrective action necessary as a surge capacitor across relay K1022 was renewed and the system was rendered operable.

Failure Data: This is the first occurrence of an event of this type.
