Earker Nature of Defect The signal from the load sequencer for the service water pu=p speed switch to go to high speed on loss of off-site power was a momentary contact. The speed switch operated, but then dropped back to low speed rhen the contact dropped out. This deficiency was discovered and documented during routine testing.
The safety hazard which could be created, is possible inadequate service water flow.
Date Information Obtained A written report indicating this tc be potentially reportable, was received by the Quality Assurante Division on December 4, 1980, from GAI Engineering.

Corrective Action The circuitry has been redesigned to include a seal-in interlock. We have determined that this condition do'es not exist on any.of our other design pr:jects. No further action is planned.
