VIRGIL C. SUMMER NUCLEAR STATION DOCKET NO. 50/395 OPERATING LICENSE N0. NPF-12 INTERIM PART 21 REPORT (SSH 920001)
South Carolina Electric & Gas Company (SCE&G) submits this letter in accordance with 10CFR21.21(a)(2) as an interim report of an identified defect potentially associated with a substantial safety hazard.
On November 7, 1991, the "C" Feedwater isolation Valve (FWIV) actuator (XVG-1611C-0-FW) failed during post modification testing.
The failure has been tracked to a " poppet" seal in the control valve assembly which controls the opening and closing of the FWlV. Subsequently, several additional failures occurred during extended testing. As a result, a new design " poppet" seal was extensively tested and placed into service, resolving the problem.
At this time. SCE&G is proceeding to evaluate the failures as follows:
1.
Performance of testing by an independent lab to determine physical properties of the original " poppet" seals.
2.
Compare "as found" physical properties of the " poppet" seals to "as specified" properties.
3.
Perform " finite element" analysis on the original and new " poppet" seal designs.
4.
Develop conclusions as to 10CFR21 reportability.
.SCE&G expects to complete tnis evaluation by July 15, 1992, due to the tieed for vendor input.
