Nature of Defect, Substantial Safety Hazard Created and Evaluation As a design inprovement, current limiting fuses were to be placed in series with a DC power circuit located in one of the main control board termination cabinets. Later, the architect engineer justified separation of less than six (6) inches by analysis as allowed in IEEE 384-1974, section 5.6.2.

We analysis took credit for the fuses installed in the circuit as adequate overcurrent protection against the possibility of an electrically generated fire due to conductor overheating. During an inspection of the fused feeder circuits in the main control board termination cabinets, it was discovered that these fuses were not correctly installed because the architect engineer had not revised the termination sheet used by field personnel to do the work.
