SUMMARY

OF OPERATING EXPERIENCE MONTHNEAR: April 1999 The following chronological sequence by unit is a summary of operating experiences for this month that required load reductions or resulted in significant non-load related incidents.

UNIT ONE:

04/01/99 0000 Unit starts the month at 100% / 850 MWe.

04/20/99 1044 Commence unit ramp down to swap HP heater drain pump. Unit at 100% / 850 MWe.

04/20/99 1110 Stopped ramp. Unit at 98% / 830 MWe.

04/20/99 1255 Commenced ramp up. Unit at 98% / 830 MWe.

04/20/99 1434 Unit at 100% / 850 MWe.

04/30/99 2400 Unit finished the month at 100% / 850 MWe.

UNIT Two:

04/01/99 0000 Unit started the month at 76% / 660 MWe. (coast-down continues) 04/18/99 0052 Turbine tripped for Refueling Outage.

04/18/99 0103 Reactor tripped for Refueling Outage.

e e Surry Monthly Operating Report No. 99-04 Page 10 of 18 FACILITY CHANGES THAT DID NOT REQUIRE NRC APPROVAL MONTHNEAR: April 1999 FS99-002 UFSAR Change Request 04/08/99 (Safety Evaluation 99-022)

As a result of the Integrated Configuration Management Project review, UFSAR Change Request FS99-002 contains corrections and clarifications to the UFSAR sections that discuss Surry's radiation monitoring system. These changes are to enhance accuracy and do not affect any radiation monitoring system or structure, or any of its component's operation or performance.

SE 99-026 Safety Evaluation 04/13/99 Safety Evaluation 99-026 evaluates the 1999 Unit 2 Refueling Outage against current industry guidelines and Virginia power administrative requirements for shutdown risk management including NUMARC 91-06 and NE Technical Report 865, Rev. 3. The schedule provides a plan for safely and efficiently accomplishing refueling, maintenance, modification, and testing activities.

DCP 98-092 Design Change Package 04/15/99 (Safety Evaluation 99-029)

Design Change Package 98-092, "Reactor Protection Test Logic Surge Suppressors/Surry/1 &2", installed surge suppressors on certain Reactor Protection Logic test relays to prevent noise spiking during the performance of the monthly Reactor Protection Test.

TM S1-99-004 Temporary Modification 04/18/99 (Safety Evaluation 99-033)

Air compressor 1-SA-C-3 is leaking air and is unable to supply air to the Laundry Facility.

Temporary Modification S1-99-004 installed a temporary compressor to supply reliable air to the Laundry Facility.

TM S1 003 Temporary Modification 04/22/99 (Safety Evaluation 99-034)

Temporary Modification S1-99-003 evaluates the use of three fire hose stations to supply the sprinkler systems in temporary trailers in the Turbine Building to support the Unit 2 Refueling Outage.

TM S1-99-005 Temporary Modification 04/23/99 (Safety Evaluation 99-035)

Temporary Modification S1-99-005 installed a temporary Non-Safety Passive Autocatalytic Recombiner in Unit 1 Containment to maintain the containment Hydrogen concentration less than the design basis limit of 0.8%.

Design Change Package 99-035, "Fuel Assembly Repair, removes fuel assembly top nozzles containing defective parts and replaces them with new, functionally equivalent, top nozzles.

ET S-99-079 Engineering Transmittal 04/29/99 (Safety Evaluation 99-038)

Engineering Transmittal S-99-079, "Alternate Packing Material for 2-RC-77", accepts the previous 1996 injected packing of valve 2-RC-77 as a permanent repair since the packing has not leaked over the past two cycles.

Abnormal Procedures 1/2-AP-4.00, "Nuclear Instrumentation Malfunction", were changed to allow entry into containment without audible count rate available. Revision 1 added that if the count rate increases by 1h decade over its previous reading, or an increasing trend in the count rate is observed, personnel must be evacuated from containment.

O-MOP-GW-001 Maintenance Operating Procedures 04/07/99 O-MOP-GW-002 (Safety Evaluation 98-064, Rev. 1)

Maintenance Operating Procedures O-MOP-GW-001, "Purging the Outer Tank for 1-GW-TK-1A", and O-MOP-GW-002, "Purging the Outer Tank for 1-GW-TK-1 B", were revised to provide a Temporary Modification to purge and sample the outer Waste Gas Decay Tank for maintenance. Revision 1 added procedure O-MOP-GW-002 for tank 1B.

OP-23.2.11 Operating Procedures 04/06/99 OP-23.2.12 (Safety Evaluation 99-020)

Operating Procedures OP-23.2.11, "Sampling Alignment for WGDT 1A", and OP-23.2.12, "Sampling Alignment for WGDT 1B", were revised to provide a Temporary Modification to install tubing between the sample outlet for the outer Waste Gas Decay Tank (WGDT) to the local sample panel used to sample the inner WGDT.

1/2-0SP-FW-004 Operations Surveillance Procedures 04/08/99 (Safety Evaluation 99-023)

Operations Surveillance Procedures 1/2-0SP-FW-004, "Unit 1 and Unit 2 AFW and AFW Cross-tie ISi Test", were written to perform the ASME Section XI pressure testing in the AFW cross-tie line. The procedures will control the removal of the seal-in contacts on several Auxiliary Feedwater (AFW) motor operated valves to allow throttling capability.

O-MCM-0125-01 Mechanical Corrective Maintenance Procedure 04/12/99 (Safety Evaluation 99-024)

Mechanical Corrective Maintenance Procedure O-MCM-0125-01, "Main Condensate Pump Maintenance", was revised to provide a Temporary Modification to temporarily remove and re-install a section of fire protection piping which interferes with the removal of the condensate pumps/motors during maintenance.

O-ECM-2503-03 Electrical Corrective Maintenance Procedure 04/13/99 (Safety Evaluation 99-025)

Electrical Corrective Maintenance Procedure O-ECM-2503-03, "Main Generator Hydrogen System Maintenance", was revised to provide a Temporary Modification to install a temporary pressure gauge in the main generator hydrogen gas system to perform a system leak check.

Operating Procedures 1/2-0P-RC-002, "Reactor Coolant System Fill", were (Unit 2)/will be (Unit 1) revised to remove the Motor Operated Valve seal-in circuit to allow electrically "bumping" open the Loop Stop Valves from the Main Control Room when filling a drained Reactor Coolant loop. This activity will minimize the time and dose required to manually place the valve at zero deflection inside containment.

2-0P-RC-011 Operating Procedure 04/13/99 (Safety Evaluation 99-028)

Operating Procedure 2-0P-RC-011, "Pressurizer Relief Tank Operations", was revised to provide an alternate vent path for degassing the Unit 2 Pressurizer Relief Tank (PRT) to the Overhead Gas System through the PRT vapor space sample line when gaseous activity levels are high.

2-MOP-DG-002 Maintenance Operating Procedure 04/15/99 (Safety Evaluation 99-030)

Maintenance Operating Procedure 2-MOP-DG-002, "Removal and Return to Service of the PDTI Inlet PCV", was written to maintain pressure control valve (PCV) 2-DG-PCV-200 in the open position during Cold Shutdown or Refueling Shutdown in order to provide a flow path for Reactor Coolant System loop drains and other drains to the Primary Drain Transfer Tank (PDTI).

2-MOP-EP-401 Maintenance Operating Procedures 04/15/99 2-MOP-EP-402 (Safety Evaluation 99-031)

Maintenance Operating Procedures 2-MOP-EP-401, "Unit 2 Generator Tagout", and 2-MOP-EP-402, "Return to Service of Unit 2 Main Generator", were revised to provide a Procedurally Controlled Temporary Modification to bypass the fire detection alarm to the Low Pressure CO2 Fire Suppression system for the turbine and generator tagged out during the Refueling Outage.

O-MCM-1410-04 Mechanical Corrective Maintenance Procedures 04/15/99 O-MCM-1420-02 (Safety Evaluation 99-032)

Mechanical Corrective Maintenance Procedures O-MCM-1410-04, "Turbine Pedestal Maintenance", and O-MCM-1420-02, "Low Pressure Turbine Maintenance", were revised to provide a Procedurally Controlled Temporary Modification to bypass the bearing lift oil pressure anti-start interlock in the main turbine Turning Gear motor circuitry to allow periodic rotation of the turbine rotor using the turning gear.

Vendor Procedure, FP-VPA-FR1 .0.1999, "Fuel Repair for Surry Unit 1 and Unit 2", provides instructions for the installation and removal of equipment required for fuel repairs as well as for the actual fuel repairs including removal and reattachment of the fuel assembly top nozzle. To perform the repairs, the temporary replacement of the new fuel elevator basket with a Westinghouse basket will be performed via a Procedurally Controlled Temporary Modification.

SE 99-039 Safety Evaluation 04/29/99 Safety Evaluation 99-039 evaluates the "Safety Monitor", which is a program that will perform on-line risk calculations to support compliance with 10 CFR 50.65 (the Maintenance Rule).
