1. DESCRIPTION OF EVENT: With Unit No. 2

-~~aler..t!!.".and. .. "high" alarm lights on. The automatic closure.of HCV-CC-100 (CC Head Tank Vent) and the* associated annunciator had not.._func.t.ioned

2. PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:

The CC Head Tank vents. to the Process Vent*System. The process vent system has it's own radiation monitor and if the high alarm-setpoint-is..: exceeded-, -effluent releases are automatically terminated. Since CC Head .Tank Vent is monitored by the Process Vent System and the, .operator -:i.mmediately closed the CC Head Tank Vent Valve, the health and safety of the public-were not-a-ffected-.-.-

3. CAUSE: The cause was. due to a failed card in thealarm:circuit.
4. IMMEDIATE*

CORRECTIVE ACTION:* The immediate action' was. to close the CC HeacLTank Vent Valve as specified in: AP-5 ~-5. ------------*-------
