1. DESCRIPTION OF EVENT:

With Unit No. 2

  • at 100% power, an operator-=:-ohserved~the_:_~_CC-1.06 -~~aler..t!!.".and...~-~---

"high" alarm lights on. The automatic closure.of HCV-CC-100 (CC Head Tank Vent) and the* associated annunciator had not.._func.t.ioned *..This_+/-s_c.o.nt.raryc_*- - - - - -

to Tech. Spec. 3 *.7-~ Table 3. 7-5 and is- rep_g:r~able per Tech. Spec. 6.6.2.b(4) *

2. PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:

The CC Head Tank vents. to the Process Vent*System. The process vent system has it's own radiation monitor and if the high alarm- setpoint-is..: exceeded-, ---

effluent releases are automatically terminated. Since CC Head .Tank Vent is monitored by the Process Vent System and the, .operator -:i.mmediately closed the CC Head Tank Vent Valve, the health and safety of the public- were not-a-ffected-.-.-

3. CAUSE:

The cause was. due to a failed card in thealarm:circuit.

4. IMMEDIATE* CORRECTIVE ACTION:*

The immediate action' was. to close the CC HeacLTank Vent Valve as specified in:

5. SUBSEQUENT CORRECTIVE ACTION:
