UNJI ONE: 09/01/93 09/30/93 0000 The reporting period began with the Unit operating at 81 % power, 600 MWe; high pressure heater drain pump level control valve, 1-SD-LCV-106, inoperable.

0737 Started power increase following maintenance on 1-SD-LCV-106.

0930 Unit at 100% power, 775 MWe. 2400 The reporting period ended with the Unit 1 at 100% power, 820 MWe.

SUMMARY

OF OPERATING EXPERIENCE MONTH/VEAR:

September, 1993 The reporting period began with the Unit heating up in preparation for start-up. (Reactor had tripped due to safety injection relay failure on 08/27/93)

Unit on-line. Started power increase.

Stopped power increase at 97.5%, 760 MWe. Holding for evaluation of Steam Generator "C" level oscillations.

Started power reduction to maintain condenser vacuum while cleaning water boxes. Stopped power reduction at 90%, 715 MWe. Started power increase following water box cleaning.

Stopped power increase at 97.5%, 760 MWe. Started power reduction to maintain condenser vacuum while cleaning water boxes. Stopped power reduction at 95%, 740 MWe. Started power increase following water box cleaning.

Stopped power increase at 99%, 785 MWe. Started slow power increase.

Stopped power increase at 100%, 820 MWe. Started power reduction due to Steam Generator "C" level oscillations.

Stopped power reduction at 99%, 800 MWe. Started power reduction due to Steam Generator "C" level oscillations.

Stopped power reduction at 98%, 792 MWe. The reporting period ended with the Unit at 98% power, 792 MWe, due to Steam Generator "C" level oscillations.

SE 93-184 DCP 86-03 TM S1-93-11 FACILITY CHANGES THAT DID NOT REQUIRE NRC APPROVAL MONTH/YEAR:

September, 1993 Safety Evaluation e Surry Monthly Operating Report No. 93-09 Page 11 of 16 09-09-93 Safety Evaluation 93-184 was performed, following a failure of a Unit 1 "B" Steam Generator

{SG) channel head drain line weld, to assess the impact of a potential failure of other similar SG channel head drain lines. The evaluation concluded that a single high head safety injection pump could effectively provide emergency core cooling to mitigate the effects of a SG channel head drain line failure. This type of failure would result in Reactor Coolant system (RCS) leakage into the Containment and would be identified through routine Technical Specification RCS leakage surveillances and by containment gas and particulate radiation monitors.

Therefore, an unreviewed safety question does not exist. Design Change Package (Safety Evaluation No.90-195) 09-10-93 Design Change Package 86-03 replaced the manual bypass and isolation valves for the Unit 1 and 2 Instrument Air (IA) system dryers, 1/2-IA-D-1, with automatic pneumatic) valves. The new valves were installed to provide for timely automatic isolation of the IA system dryers in the event of a drop in pressure in the IA header downstream of the dryers. Specifically, the new valves are designed to allow automatic bypass and isolation of the IA dryers in the event the dryer's automatic blowdown feature does not function as designed.

This DCP also installed a new battery power supply system, including a charger with a constant voltage transformer to protect the charger from high voltage electrical spikes. These modifications increased the reliability of the nonsafety-related IA system. The automatic valves represent an IA system enhancement and perform the same functions as the manual valves. The new valves do not affect any safety-related system. Therefore, an unreviewed safety question does not exist. Temporary Modification

{Safety Evaluation No.93-185) 09-20-93 Temporary Modification

{TM) S1-93-11 installed electrical jumpers to bypass test switch 1-LC-495-TB on the Unit 1 Safety Injection (SI) system train "B" circuitry to allow its replacement without rendering the circuits inoperable.

This TM continued to allow SI system train "B" actuation on a valid signal and did not affect train "A", thus maintaining the design SI functions.

Double verification of jumper installation/removal and post maintenance testing were performed.

Therefore, an unreviewed safety question did not exist.

FS 93-45 VPAP-0102 e Surry Monthly Operating Report No. 93-09 Page 12 of 16 PROCEDURE OR METHOD OF OPERATION CHANGES THAT DID NOT REQUIRE NRC APPROVAL MONTH/YEAR:

September, 1993 Updated Final Safety Analysis Report Change Station Administrative Procedure (Safety Evaluation No.93-188) 09-27-93 UFSAR Section 12.2.2.8.2, "[Responsibility of the] Supervisor

-Quality", was revised to delete the requirement for the Supervisor of Quality to review and concur with changes to procurement documents and modifications, maintenance, and repair procedures.

Station Administrative Procedure VPAP-0102, "Station Nuclear Safety and Operating Committee", was changed to delete the requirement for a Quality Assurance member on the Procedures Subcommittee.

These changes were administrative in nature and did not modify or affect the operation of the station. Therefore, an unreviewed safety question did not exist.
