1. Description of Event During the performance of Periodic Test 24.11, it was noted that 64 of imately 1800 fire barrier penetrations in safety related areas were not properly sealed, and fire watches were not established within the allotted time period. This is contr_ary to Technical Specifications
3. 21.G.l and 3 *. 21.G. 2.a and is reportable pursuant to-Technical.Specification 6~6~2.b.4.

and 6.6.2~b.3., respectively.

2. Probable Consequences and Status of Redundant Systems \ The functional integrity of the fire barrier penetration seals ensures that fires will be confined or adequ~tely retarded from spreading to adjacent portions of.the facility.

Establishing a'fire watch adjacent to non-functional fire barrier penetration insures prompt detection of a fire. Because various penetrations were not properly sealed and fire watches not established, a fire in one area could have spread to other areas.of the facility prior to detection or extinguishm~nt.

All other fire protection systems .remained operable*, *therefore the health,and safety * ..

  • of ,the general* public were_ not affected.
  • 3o Cause 4. s. The fire barrier penetrations were not properly -sealed at the time of initial construction.

Due to inadequate administrative control, fire watches were not estaolished within one hour, as required, while the discrepqncies were evaluated.

Immediate Corrective Action Work orders to correct all discrepancies were issued immediately after completion of the procedure.

Subsequent procedure review determined*fire watches .had not been posted as required.

Fire watches-were then posted in all safety -related areas where discrepancies had been not~d during the inspection 5 days prior. Subsequent Corrective Actions All penetrations were temporarily sealed and the fire watches released.

The fire barrier penetrations will be permanently sealed in accordance with issued work orders and NFPA codes. 6. Actions Taken to Prevent Recurrence No further actions are considered necessary at this time. l Generic Implications r r All fire barrier penetrations have been inspected, and the penetrations with discrepancies have been identified and are being properly sealed. The current PT-24.11 _will be changed to preclude this situation from reoccurring.

Consequently, there are .no generic implications.
