Rod Position Indicator Inoperative (2Dl) CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -RX Power 50% -Unit Load 530 MW CONDITIONS PRIOR TO OCCURRENCE:

During normal operation on February 7, 1981, the Rod Bottom Light of Rod 2Dl on Panel 1RP3 came on and Rod 2Dl position indication meter went to zero. Since there was no reactivity transient indicative of a dropped rod, the RPI channel was declared inoperable.

Action Statement 3.1.3.2.a was entered at 1930 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.34365e-4 months <br />. The Action Statement requires that with a maximum of one rod position indicator channel per group inoperable, either determine the position of the indicating rod(s) indirectly by the movable incore detectors, at least once per eight hours, and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's position; or reduce thermal power to less than 50% of rated thermal power within eight hours. This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -The rod position indicator cable connector was making loose contact at the reactor head connection.

-ANALYSIS OF OCCURRENCE:

The cable connecting the rod head and its junction box is a seven conductor cable approximately 25 feet long. Inspection of the head connector revealed that the pins had spread. Apparently, the spread pins were causing intermittent electrical connection.
