SUPPIDENTARY IIEGMATICN Report #50-302/81-047/03IA in Facility:' Crystal River Unit 3

. m*

Report Date: August 12, 1981

Occurrence Date
- . July 15,1981 Identification'of Occurrence:-

Decay Heat Pump discharge throttle valve would not control flow: in automatic contrary _to Technical Specification 3 5 2.

Conditions Prior.to Occurrence:

Mode 1 - Power' Operation-(39%)

' Description _of Occurrence:

At 0520 during performance of SP-340, ECCS Pump Operability, it 'was discovered that decay heat pump discharge _- throttle i=

valve DHV-110 would not control flow ~in automatic.

Maintenance was initiated,~ and operability---was restored at L

'1753 Designation cf Apparent Cause:

The cause of this event is attributed to a stuck swit91 in-the flow control instrument.  :

Analysis of Occurrence: )

Redundancy was provided by ES Subsystem "B". There was no effect upon the health or. safety of the general public.

Corrective Action:

The: switch was lubricated, exercised and the functional test

- was satisfactory. ' An engineering evaluation (REI- 80-9-3) of -

the control system for DhT-110.and DHV-111.is currently in progress.

Failure Data:

This is the eighth occurrence for DhT-1.10, and this is the Leixteenth event reported under.this Specification.
