5. Identification of Occurrence:

One borated water source inoperable contrary to Technical Specifications 3.1.2.9 and

6. Conditions Prior to Occurrence:

Mode 1 operation.

7. Descriptien of Occurrence:

At 1525, while- performing the Operability of 3oren Injection Sources and Pumps Surveillance Procedure SP-320, it was determined that neither "A" nor "B" boric acid f. !

storage tanks met Tech Spec requirements. The boron ecucentracien of the "A" 3AST l

.was determined to be 14,400 PPM, 400 PPM abowl the Tech Spec H mics. The volume of )

the "3" 3AST was discovered to be 4100 gallons, 900 gallens below the Tech Spec limit. l The 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> Action Statements were entered and addition of dsmineralized water to the "A" 3AST was initiated. Sampling of the "A" BAST indicated the boren concentration

b returned to within specified limits at 2035, satisfying the Action Statements. The volume of the "3" 3AST was returned to within specified limits en 9 February 1978 as indicated by satisfactory ccepletion of SP-320.

3. Designation of Apparent Cause:

The probable cause of this event was the failure to notif 7/ initiate action then administrative limits of the boric acid storage tanks were appnached :n previous sampling.
