Facility: Crystal River Unit #3 Report Date: 22 October 1979 Occurrence Date: 15 October 1979 Identification of Occurrence:

Inadequate design of SW system restraint SWR 515, reportable under Technical Specification 6.9.1.8.1.

Conditions Prior to Occurrence:

Mode 1 power operation (100%)

Description of Occurrence:

a At 1220 it was reported that on 13 October 1979, seismic re-straint SWR 515 was found to be inadequately designed. Original computer analysis conducted by Gilbert Associates indicated a need for a horizontal restraint. During verification of com-puter analysis, it was discovered that a vert 4 cal restraint was improperly designed and installed. A modification was initiated to correct this deficiency and provide an adequate design for SWR 515.

Designation of Apparent Cause:

The cause of this event is attributed to inadequate design.

Analysis of Occurrence:

No effect upon the plant or general public.

Corrective Action:

SWR 515 was redesigned and repaired through modification 79-6-86H.

.. Repairs to SWR 515 were completed on 13 October 1979.

Failure Data:

This is the third occurrence of this type reported. Reference LER 79-061/0lT-0, 79-087/OlT-0.
