response to the item of noncompliance.

On July 14, 1980, valves 21CS6 and 22CS6 were found tagged closed, rendering

the Containment

Spray System inoperable (Technical

On July 13, 1980 valves 21MS45 and 23MS45 were found tagged closed, rendering

the steam driven Auxiliary

Feedwater

Pump inoperable (Technical

Specifications

3.7.1.2).

In each case, the tagging predated the entry into Mode 3. Reply To Item 1 A. 21/22CS6 1. The cause was an improper use of a temporary spot change, as applied to a surveillance

procedure.

with the appropriate

procedure.

3. To prevent recurrence, the Operating

Department

procedure

concerning

the use of on-the-spot

changes was revised. This revision will prohibit the use of* a temporary

on-the-spot

change to modify any surveillance

restoration

step. A Night Order Book entry was made to emphasize

the proper use of the on-the-spot

change. 4. Date for full compliance

was November 1, 1980 B. 21/23MS45

1. The cause was a personnel

error. The identified

tags were released prior to entering Mode 3, however, the tags.were

not immediately

removed nor the valves .repositioned.

2. *To correct the situation, the tags were

and the valves were locked open in accordance

with the appropriate

procedure.

A satisfactory

surveillance

was performed

on No. 23 Auxiliary

Feed Pump. 3. To prevent recurrence, the overall Operating

Instruction

was revised to remind the Operators

to perform this surveillance

after entering Mode 3. Administrative

Procedure

-Safety Tagging Program (AP-15) was revised to require a log entry by the Senior Shift Supervisor/

Shift Supervisor

when a tagging request is not released upon sign-off by the supervisor

completing

the work. 4. Date for procedure

revision was November 1, 1980. A more thorough review of the item of noncompliance

and initial response revealed that a revision of Administrative

Procedure

15 did not completely

correct the problem, therefore, the following

preventive

action will be taken: To ensure that all equipment

required for changing to Modes 4 and 2 is available, a review of all outstanding

Tagging Requests will be completed.

This review ment has been incorporated

in the mode change checkoff sheets.
