Two Auxiliary Building Exhaust Fans Inoperable CONDITIONS PRIOR TO OCCURRENCE:

At 0935 hours0.0108 days <br />0.26 hours <br />0.00155 weeks <br />3.557675e-4 months <br />, No. 13 Auxiliary Building exhaust fan was removed from service due to high motor vibration.

No. 11 Auxiliary Building exhaust fan had been previously removed from service due to problems with the fan unit dampers. Action Statement C for Technical Specification 3.7.7.1 was implemented.

At 1700, surveillance 4.7.7.l(A) had been performed on the repaired No. 13 fan unit to verify operability and the Action Statement was terminated.

DESIGNATION.OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence was a worn keyway in No. 13 exhaust fan motor shaft. ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.7.1 Action C requires that with only one exhaust fan operable, restore at least two exhaust fans to operable status within seven days or be in at least Hot Standby within the next six hours and in Cold Shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.
