Pursuantto10CFR21,thisletterisbeingsubmitted toprovidewrittennotification oftheexistence ofadefectassociated withmotor-operatedvalves(MOVs)suppliedtotheD.C.CookNuclearPlantbyHammel-Dahl 6Jamesbury ControlsCompany,Inc.throughMidwestControlsCompany,Inc.Initialnotification wasmadeinatelephone conversation be'tweenrepresentatives oftheAmericanElectricPowerServiceCorporation andMr.D.C.DiIannioftheNRCOfficeofNuclearReactorRegulation onAugust21,1987.OnAugust13,1987,duringtheinstallation offourmotor-operated valvesintheAuxiliary Feedwater SystemattheD.C.CookNuclearPlant,metallicbeadswerefoundinthevalvemotorcasings.Thebeadsappearedtobeofthetypeassociated withanabrasivecleaningprocess.Thebeadswerefoundinoneofthemotorsduringinstallation ofT-drainfittingsforthemotorcasings.Noisebeingmadebytheva"vemotorduringMOVATS(motor-operated valveanalysisandtestsystem)testingprompteddisassembly ofthatmotor.Duringthesubsequent investigation fortheremaining Unit1ACmotors,beadswerefoundintwoofthethreemotors.Thevalvemotors(575VAC)weremanufactured byRelianceElectricCompanyandsenttoLimitorque Corporation tobeinstalled onvalveoperators (Limitorque SMB-000).
Theentiremotor-operator assemblies werethenshippedtoHammel-Dahl
&Jamesbury ControlsCompany,Inc.,wheretheyweremountedtothevalves.Thevalves,withthemotoroperators
- attached, werereceivedbytheplantinMay1987andinstalled duringthecurrentoutage.Aftertheproblemwasdiscovered, itwasanalyzedbyAmericanElectricPowerpersonnel, andonAugust21,1987,itwasconcluded thatthebeadsfoundinthevalvemotorsoftheMOVsweresufficient toL8709040009 870826PDRADOCK05000315IiPDR

Thisconclusion wasbasedonthefactthatwedetermined that:tKhbeadQPcM3ld haveeitheraffectedthemechanical rotationofthemotororcreatedthepossibility ofthebeadswearingintothewindinginsulation, thuscausingfailureofthemotorsandtheelectrical inoperability ofsafety-related valves'similarproblemexistswithatleastoneofthevalvemotorspurchased butnotinstalled forUnit2(theUnit2valveswerescheduled tobeinstalled duringthenextrefueling outage).Sixofthedefective motorshavebeenreturnedtoLimitorque viaRelianceElectricCompany.Theremaining twohavebeenreturneddirectlytotheLimitorque Corporation.
Wehavereceivedsixrefuibished motorsfromLimitorque andarepresently installing fouroftheseonUnit1.Wearenotawareofotherfacilities oractivities subjectto10CFR21forwhichthesevalvemotorshavebeensupplied.
