REPORT NO. 2 (REVISION OF CORRECTIVE ACTIONS IN THE FINAL)

NCR'S S-7 AND S-13 On January 15, 1980, TVA informed NRC-0IE, Region II, Inspector R. W. Wright, of a potentially reportable condition under 10 CFR Part 50.55(e) concerning improper exposure and evaluation of radiographic examination (RT) film on weld buildup edges (buttering) of the Hartsville unit B-1 drywel2 vent structure and Phipps Bend unit 1 reactor pressure vessel (RPV) pedestal by McManus Inspection Service of Milwaukee, Wisco ,in . Mr. Wright was

~ i also notified on March 25, 1980, of an inconsistency between the corrective action noted in our final report (on the reportable nonconformance) dated -

February 15, 1980, and the corrective action which has been approved by the technical engineer to correct defects in drywell vent segments at the Hartsville Nuclear Plant. This revision to the final report is filed to correct that inconsistency. All other items in the final report are unchanged.

Revised Corrective Action All buttered edges on the RPV pedestal segments noted in NCR S-13 at Phipps Bend have been reexamined and repaired such that they are now acceptable for use.

At the Hartsville Nuclear Plant, the repairs in the drywell vent structure on unit B-1 will be performed as shown below:

Vertical Welds - Excavate rejectable defects in the weld prep using overlays from vendor radiographs. Visual and magnetic particle inspect excavations. Repair to original configura*. ion by weld overlay. Perform visual, magnetic particle, and radio-graphic examination on repair areas.

Horizontal Welds - Excavate rejectable defects in the weld prep

~

- (using overlays from vendor radiographs) to a depth of 1/2 inch into plate edge to ensure removal of defects. Visual and magnetic particle inspect excavations. Restoratior. of excavated areas will be accomplished as part of the installation weld

' joining the structure to the tase plater The final weld as -

deposited will be accep, ted in accordance with original specifications. i Vendor film quality, th'ough unacceptable per AWS criteria, is sufficient to produce repair overlays. j
