HARTSVILLE NUCLEAR PLANT A - REPORTABLE DEFICIENCY - C. F. BRAUN CLOSEOUT OF FINAL DESIGN CHANGE CONTROL DOCUMENTS -
HTRD-50-518/83-05, -520/83-05 The subject deficiency was initially reported to NRC-0IE, Region II, Inspector P. Fredrickson on March 2,1983, as NCR HTANEB8301. In compliance with paragraph 50.53(e) of 10 CFR Part 50, we are enclosing first interim report on the subject deficiency. We consider 10 CFR Part 21 applicable to this nonconformance.
TVA has elected to defer construction activities for this project.
Deferral does not mean that the project will be cancelled but that TVA is minimizing its expenditures and construction efforts until such time that TVA has sufficient information to indicate whether the project should be completed or cancelled. Therefore, we will not be submitting a final report for this nonconformance until a final decision is made regarding this project.

---HTRD-50-518/83-05, -520/83-05 Description of Deficiency The C. F. Braun (CFB) documentation control program is deficient in that it failed to adequately monitor closecut of their final design change control (FDCC) documents. The CFB FDCC log which indicates completion status of FDCCs has been found to inaccurately reflect the status of FDCCs.
An example of this program is documented by STRIDE design deficiency report (SDDR) No. CFB-33 The problem explained in SDDR No. CFB-33 was discovered by TVA as part of the STRIDE wind-down (SWD) review. As part of the SWD, TVA and CFB reviewed a total of 87 FDCCs in an effort to evaluate the extent of this problem. Of the 87 FDCCs CFB reviewed 67 to verify their closed-out status and found one other problem like that referenced in SDDR CFB-33 In this second case, FDCC-6661 was indicated in the CFB FDCC log as being closed out; however the document it was issued against (S-056) had not been revised. CFB's review was incomplete in that it consisted of checking only the primary document for which the FCDD was written. A complete review would have also involved a review of all other affected drawings listed on the cover sheet of the FDCC.
- Also, the CFB document control program called for TVA to receive copies of FDCCs stamped " Closed-Out." The CFB FDCC log was to be utilized to determine which FDCCs had been closed out. Many of:the copies stamped
" Closed-Out" and transmitted to TVA were not actually closed out according to the CFB FDCC log. Examples of these FDCCs are FDCC 4593,
-5859, -6139, and -6715.
Interim Progress TVA's investigation into this NCR shall resume upon restart of design and construction of the Hartsville Nuclear Plant A.
