OnJanuary14,1997,inevaluating thefailureof2-WIVIO-728 andtheeffectsofthemissingspaceronthetorqueswitchassembly, itwasdetermined thatthelackofthespacerallowsthetorqueswitchshafttorotateslightlyalongtheshaftlength.Thisshaftrotationallowedthetorqueswitchcontactstoopen.Withtheclosedtorqueswitchcontactsopen,theactuatorcontrolcircuitwillnotallowtheactuatortoruninthecloseddirection andtherefore willnotallowthevalvetofullyclose.Theactuatorcontrolcircuitusesonlytheclosedsetofcontactsandtherefore theopenoperation isnotaffected.

Withtheexception ofthe2-CMO-415, allthevalvesinwhichthedefective assemblies hadbeeninstalled hadeitheran"open"safetyfunction(2-WMO-728 and1-VVNlO-724) orwereaBOPvalve(2MNt0-421),
andthustheiroperability wasnotinquestion.
Duringtheinvestigation ofthisevent,anoperability evaluation wasperformed for2-CMO-415.
Thevalvewasfoundtohavebeensuccessfully testedunderdynamicconditions duringtheSeptember 1996toJanuary1997intervalandthatthedefective torqueswitchassemblywouldnothavekeptthevalvefromperforming itsdesignbasisfunctionofclosing.Therefore, thevalvehadneverbeeninoperable.
TheENSnotification wasretracted onFebruary22,1997.fnThedefective lotof7receivedfromLimitorque apparently resultedfromamanufacturing/assembly errorattheLimitorque facility.
EffFilnPIr'nThedoublepoletorqueswitchisastandardconfiguration forAmericanElectricpowerandisusedextensively attheDCCookPlantinSMB-00actuators.
The7assemblies suppliedunderASP18787wereplacedinopenstockforuseinthenextvalverequiring one,Therefore, the3torque 61AR.-19'7(%OI 09:20GOOK-KTEL:616906

Becauseitisnotpossibletodefinitively evaluateallpossibleapplications forsignificant safetyhazard,itwasconservatively decidedtosubmitthiseventasaPart21.iAnAll4ofthedefective assemblies whichwereinstalled havebeeneitherreplaced, orrepairedbyadditionofaspacer.All7assemblies werereturnedtoLimitorque forevaluation onMarch10,1997.Limitorque hasnotbeenabletopredictwhentheirevaluation mightbecomplete.
