Dear Mr. Grier:

In accordance with Technical Specification 6.9.1.8.1, the following occurrence is reported. The report was discussed with Mr. D. A. Beckman at 0930 ho2rs on July 24, 1979.

During performance of the baseglate inspection procedure developed to comply with IE 3ulletin 79-02, an inoperable baseplate was observed. The pipe hanger baseplate was H-208 which is on the three inch charging line near the regenerative heat exchange: in containment. The concrete expansion anchors in the baseplate were improperly installed and failed during the pullout test because of oversizad holes.

The failure was attributed to the lack of a formal installation procedure 6 ring the initial installation. The hanger will be redesignem and repaired or cc7 1 rc.ed prior to the plant returning to power. A complete report of inspection results and repairs will be made in the response to IE Bulletin 79-02.
