On September 13, 1989 while performing a paperwork closeout review of a Maintenance Work Request (MWR) it was discovered that the *B" Chilled Water Pump (XPP-48B-VU) had been returned to service without performing an adequate Section XI of the ASME Boiler and Pressure Vessel Code required post maintenance operability verification test.
Technical Specification Surveillance Requirement 4.0.5 requires that all inservice testing of ASME Code Class 1,2, and 3 pumps shall be performed in accordance with Section XI. Contrary to this requirement XPP-48B-VU was returned to service on September 4, 1989 after the pump was torn down and reassembled for mechnical seal replacement without verifying its operability through adequate testing.
Upon discovering that an inadequate post maintenance operability verification testing had been performed on the "B" Chilled Water Pump, the pump was declared inoperable and an Off-Normal Occurrence report was generated. The pump was then tested to confirm operability and was returned to service. There is no adverse consequence due to this event because testing did confirm that the pump was operable when it was initially returned to service on September 4, 1989.
The missed post maintenance operability verification testing is attributed to personnel error. There was inadequate review of the scope of the maintenance activity which resulted in not identifying the required testing to return the pump to service.
This event will be discussed with maintenance and operations personnel and a copy of l            this LER will be placed in the required reading file for review by those personnel, l
This action is anticipated to be completed by February 28, 1990.

CONDITION PRIOR TO EVENT:
Mode 1 - 100% Reactor Power DESCRIPTION OF EVENT:
On September 13, 1989 while performing a paperwork closeout review of a Maintenance Work Request (MWR) it was discovered that the "B" Chilled Water Pump (XPP-488-VU) had been returned to service without performing an adequate Section XI of the ASME Boiler and Pressure Vessel Code required post maintenance operability verification test. Due to the complexity of the issue regarding Section 4.0.5 of the Technical                                          I Specifications and Section XI of the ASME Boiler and Pressure Vessel Code an indepth and lengthy review this event was performed inorder to ascertain reportability.
CAUSE OF EVff_T:
The missed post maintenance operability verification testing is attributed to personnel error. There was inadequate review of the scope of the maintenance activity which resulted in not identifying the required testing to return the pump                                    !
to service.
ANALYSIS OF EVENT:
There is no adverse consequence for this event because the operability verification testing performed on September 13, 1989 confirmed that the pump was operable when it was initially returned to service on September 4, 1989.
IMMEDIATE CORRECTIVE ACTION:
: 1. On September 13, 1989 upon discovering that no post maintenance operability verification testing had been performed on the "B" Chilled Water Pump the pump was declared inoperable and an ONO report was generated.
: 2. The pump was tested to confirm operability and was then returned to service.
ADDITIONAL CORRECTIVE ACTION:
This event will be discussed with maintenance and operations personnel and a copy of this LER will be placed in the required reading file for review by those personnel. This action is anticipated to be completed by February 28, 1990.
