EVENT DESCRIPTION AND PROBABLE CONSECUENCES h I o l: lI The Pressurizer Safety Valve Position Indication on valves XVR-8010-A/B/C failed l

Lo p l l to function properly during the performance of a Channel Calibration. The Main l

lo (4 l l Control Board indication of valve position was received during the sinulatal valve l jol5ll opening; however, the indication did not change once received. There were no l

j o j e l l adverse consequences since alternate nethods of monitoring the valve position were l lo;7ll availabls for Operations personnel if the valves had opened while in Itdes 1, 2, j

o CAUSE DESCRIPTION AND CC RRECTIVE ACTIONS h l t l o l l The cause of the switch fail'ure was excessive current flow during contact l

l t l i l l closure. The current was a result of the discharge of the cable capacitance.

l l t ! 2 i l The switch assenblies were replaced, and the circuit is being mcdified to I

i j nclude a current limiting resistor nearby the switches.

'Ihe modification has j

,,,, ; been tested and wil'1 prevent a future recurrence of this failure.
