O'Reilly LER No.83-057 Page Twc July 1, 18 3 EVENT DESCRIPTION AND PROBABLE CONSEQUENCES On June 6,1983, at 1305 hours0.0151 days <br />0.363 hours <br />0.00216 weeks <br />4.965525e-4 months <br />, with the Plant in Mode 1, Nitrogen Low Pressure Alarm 1x1 the Low Pressure Accumulator for Pressurizer Power Operated Relief Valve (PORV) POV-445A-RC was received in the Control Room. PCV-445A-RC would not open from the Main Control Board. Within 15 minutes af ter receipt of the alarm, the block valve for POV-445A-RC (8000A) was closed and power removed.

Technical Specification 3.4.4, Action Statement (a), requires that with one (1) or more PORV(s) inoperable, within one (1) hour either restore the PORV(s) to OPERABLE status or close the associated block valve (s) and remove power from the block' valve (s); otherwise, be in at least HOT STANDBY within the next six (6) hours and in COLD SHUTDOWN within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

In addition to the above referenced Specification, Technical Specification 3 3.3.6, Table 3 3-10, Item 20, is also applicable but not required when the associated block valve is closed.

There were no adverse consequences as the Plant was in compliance with the applicable Technical Specification Action Statement, and the redunaant train was OPERABLE.

CAUSE AND CORRECTIVE ACTIONS The cause of the occurrence has been attributed to low operating pressure for PORV POV-445A-RC. In addition to the above problem, it was determined that the valve position indicator lights on the Main Control Board were not showing full actual position. The cause of this second problem has been attributed to the valve limit switches being out of adjustment.

Nitrogen Supply Header Pressure Control Regulator (IPV-6307A) for PORV POV-445A-RC was adjusted to allow full valve stroking. The valve limit. switches were adjusted to give proper Main Control Board Indication. Before being returned to' service on June 19, 1983, at 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br />, a Surveillance Valve Operability Test was performed satisfactorily.

No further action is planned other than normal surveillance testing.
