Dear Mr. O'Reilly,

The following report of Abnormal Occurrence No. 3-2-16 is provided pursuant to the requirements of Section 6.12.2(a) of the Technical Specificati6ns to Facility Operating License No.

DPR-26.

On November 16, 1973 an analysis of the results of periodic tests and calib~ation checks relating to pressurizer pressure in-dicated that the~setting for one of the three bistables associated with low pressurizer pressure safety irrjection was b~low that re-quired by Item 3 in Table 3-1 of the Technical Specifications.

At the time of these periodic tests and calibration checks the re-actor was in the cold shutdown condition for maintenance work.

The particular bistable device is par~ of a two out of three logic system which provides for a safety injection signal on coin-cidence of low pressurizer pressure and low pressurizer level.

The other two bistables of the system were found to be operating correctly. The channel associated with this device was recali-brated and retested satisfactorily.

Mr. Don.Johnsorr of your office was notified of this occurrence by Mr. John Makepeace by telephone on November 16, 1973.

Very truly yours, w~l-~1l~\ ~- (1 L1~<:1 - * ~
