2.0 RESPONSIBILITIES The Shift Supervisor (SS)/On-Site Emergency Coordinator (OSEC)and Fire Brigade members are responsible for the accomplishment of emergency actions required in response to a fire at DCCNP.3.0 APPLICABIIITY This procedure describes emergency fire activities including assessment and emergency classification, notifications and reporting, activation of the Fire Brigade or other emergency organizations, fire suppression activities, assistance requests to local fire departments, personnel safety, radiological considerations and response to protection.

4.0 INSTRUCTIONS 4.1 Shift Supervisor
 
-Immediate Actions 4.l.1" If possible, in a timely manner, confirm and determine the extent of the fire.4.1.2 Sound the plant fire alarm, make a P.A.announcement of the fire's location, and establish fire announcements on the fire conference circuit (telephone number 1800).4.1.3 Brief and dispatch a fire brigade leader to assume control of the fire brigade at the fire.4.1.4 Request,, if needed, that Security brings the F're Truck to the fire area.4.1.5 If the fire is within a controlled area, request assistance from the Radiation Protection Section to determine the following:

2.0 RESPONSIBILITIES 2.1 The OSEC is responsible for implementation of this procedure upon notification of injured personnel.

PMP 2080 EPP.009 INDIANA 6 MICHIGAN ELECTRIC COMPAiW DONALD C. COOK NUCLEAR PLANT FIRE EMERGENCY GUIDELINES 1.0 OBJECTIVES This procedure sets forth those actions which must be taken during emergency situations involving the outbreak of fire at on-site plant locations.

2.0 RESPONSIBILITIES The Shift Supervisor (SS)/On-Site Emergency Coordinator (OSEC) and Fire Brigade members are responsible for the accomplishment of emergency actions required in response to a fire at DCCNP.

3.0 APPLICABIIITY This procedure describes emergency fire activities including assessment and emergency classification, notifications and reporting, activation of the Fire Brigade or other emergency organizations, fire suppression activities, assistance requests to local fire departments, personnel safety, radiological considerations and response to protection.

4.0 INSTRUCTIONS 4.1 Shift Supervisor - Immediate Actions

4. l. 1" If possible, in a timely manner, confirm and determine the extent of the fire.

4.1.2 Sound the plant fire alarm, make a P.A. announcement of the fire's location, and establish fire announcements on the fire conference circuit (telephone number 1800).

4. 1.3 Brief and dispatch a fire brigade leader to assume control of the fire brigade at the fire.

4.1. 4 Request,, if needed, that Security brings the F're Truck to the fire area.

4.1.5 If the fire is within a controlled area, request assistance from the Radiation Protection Section to determine the following:

A. Radiation levels B. Airborne activity levels C. Contamination levels 4.1.5.1 The Fire Brigade Leader should instruct the fire br'gade on precautions to be taken if contamination or radioactive smoke are encountered.

PMP 2080 EPP.009 4.1.5.2 The Fire Brigade Leader should minimize the number or personnel involved and stay times when combatting fires in potential radiation hazard areas.

4.1.5.3 Fire fighters should be aware of the possible loss of radiological shielding during a fire.

4.1.6 Notify the Plant <tanager or Operations Superintendent.

4.1.7 Establish communications between the Shift Supervisor (Control Room) and the Fire Brigade Leader.

4.1.8 Direct other Plant staff members required to support the fire-fighting effort in the tasks required.

4. 1.9 Request Outside assistance if required, and notify Plant Security to meet the incoming units and escort them to the fire area.

4.1.10 Direct all on-site fire-fighting operations, including de-energizing electrical equipment and mechanical equipment, as necessary.

4.1.11 Determine need for additional support such as Reentry and Rescue Team and off-site medical support.

4.1.12 Refer to and implement as required PUP 2080 EPP.001, Emergency Plan Activation and Condition Classification, and review Emergency Action Levels based upon reports

,from the fire scene.

4.1.13 'Notify the NRC, if required.

Direct fire fighting effort until fire is out or relieved by the OSEC.

4.2 Shift Supervisor - Subsequent Actions 4.2.1 Restore Fire suppression equipment and alarm systems when the fire is out.

4.2.2 Ensure all personnel and equipment are monitored for contamination when leaving a fire area within the Controlled Area.

4.2.3 Investigate the cause of the fire and document findings per OHI 2271, Fire Investigation.

4.2.4 Ensure completion of initial off-site notification list from PMP 2080 EPP.012.

INDIANA 8 MICHIGAN ELECTRIC COMPANY PMP-2080 EPP.014 DONALD C. COOK NUCLEAR PLANT PERSONNEL INJURY 1.0 OBJECTIVES This procedure provides general guidance to the On-Site Emergency Coordinator (OSEC) to ensure that injured personnel receive appropriate first-aid; that prompt and correct notification of Memorial Hospital takes place; and that appropriate members of the Plant Manager' Safety Investigation Comnittee are notified of the event.

This procedure is supplemented by PMP-2081 EPP.008 when the injury occurs in an area where there is a potential for radiation/contamination.

2. 0 RESPONSIBILITIES 2.1 The OSEC is responsible for implementation of this procedure upon notification of injured personnel. The S;S shall act as the OSEC until relieved of this responsibility as described in PMP-2080 EPP.015 2.2 An Asst. Shift Supervisor (A.S.S.) will normally be responsible for coordinating activities at the scene of the injury. The " S.S will be assisted by someone trained in first aid as well as an individual trained in radiation protection when radiation/

contamination is involved.

2.3 Radiation Protection personnel are responsible for coordinating Health Physics activities described in PMP-2081 EPP.008.

Note: In general, the order of medical treatment will be:

1. Care of severe physical injuries
2. First aid to other injuries
3. Personnel decontamination
4. Definitive medical treatment 3.0 APPLICABILITY This procedure is applicable any time one of the following personnel injuries occurs on site:

3.1 An injury requiring transportation to the hospital in an ambulance.

3.2 Any time an injury involves transportation of contaminated individual to the hospital.

PMP-2080 EPP.014 4.0 INSTRUCTIONS 4.1 Ensure that a team is dispatched to provide medical treatment to the injured.

4.2 If radiation/contamination is involved, notify R.P. to assist as per PMP-2081 EPP.008 4.3 Obtain information necessary to complete Exhibit A and/or Exhibit B.

4.4 If an ambulance is necessary, notify Security to request local ambulance service.

4.5 If contaminated patients are involved, and an ambulance is to be used, request Security to bring the plant ambulance to the vehicle search portal for possible off-site agency use.

4.6 Notify Memorial Hospital as per Exhibit A and/or Exhibit B.

4.7 Be prepared for Memorial Hospital to call back to verify the existence of contaminated patients if Exhibit 8 is used.

4.8 If contaminated patients are involved ensure at least one member of Radiation Protection accompanies the individual to the hospital.

4.9 Ensure. appropriate plant staff notifications as per PMSO .048, Occupational- "Inj.uries.

4.10 Return to PMP-2080 EPP.001 as appropriate.
