Report No.: 50-302/78-074/03L-0 Facility: Crystal River Unit #3 Report Date: 19 January 1979 Occurrence Date: 29 December 1978 Identification of Occurrence:
Quadrant Power Tilt exceeded the steady state limit contrary to Technical Specification 3.2.4, Table 3.2-2.
Conditions Prior to Occurrence:
Mode 1 power operation (74%), RCP-1B secured (due to low oil level).
Description of Occurrence:
At 0032, during performance of Heat Balance Calculation Surveillance (SP-312), it was discovered that a quadrant power tilt existed.
Three pump operation resulted in an improper ratio of feedwater flow. Feedwater was re-ratioed and the Delta T cold brought to zero. This condition resulted in a quadrant power tilt of 3.66 in the X-Y quadrant. The maximum quadrant power tilt reached 3.71.
It was discovered that the temperature channel used for Delta T cold control was selected to the idle loop. Upon switching to the proper channel and re-ratioing feedwater flow, the quadrant power tilt returned to within acceptable limits.
Designation of Apparent Cause:
The cause of this occurrence is attributed to the operator not selecting the proper temperature channel used for Delta T cold control.
Analysis of Occurrence:
The health and safety of the plant and general public were not endangered as surveillance was increased to closely monitor and control this event.
Corrective Action:
Revision to Operating Procedure 302, RC Pump Operation, is being implemented to prevent recurrence.
Failure Data:
Two previous occurrences of this type.
