Operational Mode 3 Reactor Trip Recovery DESCRIPTION OF OCCURRENCE:
At 1258 on 12-13-78, during recovery from a reactor trip, No. 13 Auxiliary Feed Pump started as required, but tripped on overspeed.
At 1258 on 12-13-78, during recovery from a reactor trip, No. 13 Auxiliary Feed Pump started as required, but tripped on overspeed. No. 11 and 12 Auxiliary Feed Pumps operated satisfactorily. The Action Statement for Technical Specification 3.7.1.2 was implemented and Performance Department was notified. At 2045, functional tests showed operation of the turbine governor to be unsatisfactory. A Woodward Governor field representative was requested for on site technical assistance during troubleshooting and rework of the turbine controls. At 2010, 12-14-78, cooldown of the plant commenced to facilitate planned maintenance activities requiring the unit to be in Cold Shutdown. The unit entered Mode 4 at 0515 and entered Mode 5 at 0858 on 12-15-78. At 2325 on 12-19-78, a functional test of No. 13 Auxiliary Feed Pump showed proper operation of the governor controls and the Ac.tion Statement was terminated.

Troubleshooting the governor assembly showed that the operating speed was set at 4200 rpm, resulting in excessive overshoot during starting and subsequent tripping on overspeed.

While troubleshooting of the governor was in progress, additional maintenance was found to be required on secondary components and the unit was taken to the Cold Shutdown mode. No. 11 and 12 Auxiliary Feed Pumps provide redundant operation during this occurrence.
Technical Specification 3.7.1.2 requires that with one Auxiliary Feed-water Pump inoperable, restore at least three Auxiliary Feedwater Pumps to operable status within 72 hours or be in Hot Shutdown within the next 72 hours. While troubleshooting of the governor was in progress, additional maintenance was found to be required on secondary components and the unit was taken to the Cold Shutdown mode.
