Subject:

INDUSTRIAL ACCIDENT RESULTING IN DEATH At approximately 10:00 a.m., September 15, 1988, a licensee employee died from apparent anoxia or drowning. The victim had jumped into a condensate storage tank in efforts to assist a diver who had fallen from an internal ladder before having donned his breathing apparatus while entering the tank for maintenance. The tank is under a nitrogen blanket. The diver was promptly recovered from the tank by a third employee by means of the diver's safety line, administered first aid and evacuated to a local hospital. The victim's body was recovered by state police divers at approximately 1:00 p.m.

The State of Maryland has been notified. The licensee is issuing a press release.

NRC Region I has notified OSHA. The resident inspectors are following licensee investigative efforts and conducting independem inspections.
