MINUTES FROM BALTIMORE GAS AND ELECTRIC PRESENTATION OF THE CALVERT CLIFFS PERFORMANCE IMPROVEMENT PLAN - 08/04/89 A public meeting was conducted between Baltimore Gas and Electric (BG&E) and j' the NRC staff on August 4, 1989, to accommodate the formal presentation of the Calvert Cliffs N rformance Improvement Plan Implementation Program by BG&E. The

! meeting was held at NRC Headquarters, Rockville, Maryland. A list of attendees is enclosed.

After brief opening remarks by Mr. Boger, NRC and introduction of attendees, Mr. George Creel, BG&E, began the presentation with a outline of the meeting purpose and agenda. Mr. Creel introduced Mr. Chris Poindexter, the new Vice Chairman of BG&E, who made brief remarks on his background and his activities with regard to the Performance Improvement Plan (PIP) and the new PIP Implementation Program (PIPIP). He stated that these were long-term plans, providing leadership for the future of BG&E.

Mr. Creel then continued with a presentation of the PIP objective; to have

" event-free" operation with focused actions on addressing causes of performance decline, complementing improvements currently underway, improve problem resolution and monitor overall progress. He stressed that the PIP was not a short-term plan, and that it is not their intention to stop performance improvement efforts at some future point.

Mr. Robert Denton, BG&E, discussed the Performance Improvement Phases. He outlined Phase 1, the development of the PIP, which resulted in its submittal on April 7, 1989. Phase 2, Implementation of the PIP, the PIPIP, details development of PIP action plans, management and verification. This was detailed in the July 31, 1989 submittal. The development of the PIP was f detailed in three stages, 1) creation of the symptom database, gleaning information on performance from the last two years, 2) arriving at a fixed set of Root Causes which address the symptoms, and 3) development of a plan of corrective actions to address the Root Causes. The root causes were further grouped into three categories, 1) expectations, accountability and i communications, 2) problems correction, and 3) resource allocation. '

Mr. Charles Cruse, BG&E, detailed the action plan progress thus far for Operations and Maintenance, and Engineering areas. For Operations and Maintenance, BG&E has combined operations and maintenance organizations, integrated work activities and adjusted work priorities. For Engineering, system engineers have been moved inside the security area, have had their job description revised, and a training program has been set up. Engineering j planning has been improved and procedure upgrade project is in progress.

Mr. Richard Olson, BG&E, addressed.the PIP Implementation Program (PIPIP). He l discussed the basic construction of the PIPIP, its relation to the PIP and the 1 associated Root Causes. An example was discussed in detail how a symptom was identified, connected to a set of root causes, through the PIP, and finally tracking with the PIPIP verification process. He further outlined the appendixes to the PIPIP, discussing the PIP summary status report and histograms. j The NRC asked several questions in order to clarify points during the symptom example discussion. These questions involved the differences between the l :

various Root Causes and why they seem somewhat similar. Eventually, Mr. Olson ,

reiterated the entire PIP construction process in more detail, discussing the '

supervisory and engineering support. Further discussion was held on the list  ;'

' of Root Causes, and how the final list was determined.

Mr. Olson then discussed the PIP verification nrocess. This process consists I of three steps, 1) implementation verification, 2) management feedback verification, and 3) effectiveness verifict.cion. in the area of effectiveness verification, BG&E would use the vertical slice inspection techniques through the use of four assessment and inspection types,)1) events analysis, management 3 design and quality and implementation assurance, processes, and 2) self-

4) maintenance / operations interface and support. He stressed that BG&E expects to see improvements and would adjust the PIPIP as necessary to get positive results.

Mr. Creel summarized the presentation by discussing BG&E resource allocations.

To types of resources would be used, one to create the new programs, and the second to maintain those programs. Verification would be performed by vertical slice audits, performance objective reviews, and the BG&E Nuclear Program Plan.

The PIP /PIPIP would remain flexible and would not consider a checklist. He further stated that the PIP is a long-term performance improvement plan, and would be institutionalized at Calvert Cliffs.

Upon completion of the presentation, the NRC asked how recent managerial changes would be reflected in the PIP. When asked how far down the chain of command the PIP was communicated, BG&E stated that their managers have been conducting

" focus" meetings with small groups and meeting routinely with groups at lunch and at shift meetings.

After closing remarks by Mr. Poindexter and Mr. Boger, the meeting was adjourned.
