APPENDIX A NOTICE OF VIOLATION .

Baltimore Gas and Electric Company Docket Nos. 50-317; 50-318 Calvert Cliffs Nuclear Power Station License Nos. DPR-53; DPR-69 As a result of the inspection conducted on June 16-20, 1986, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

A. Technical Specification 6.8.1 requires written procedures to be estab-11shed implemented and maintained for activities including surveillance tests, and maintenance.

Contrary to the above, as of June 20, 1986, the implementation of procedures affecting safety-related activities were inadequate as evidenced by the following examples:  !

The technicians who performed safety-related work under Maintenance Orders 205-347-209A, 205-141-984A and 206-070-337A neither documented the component qualification number on the instruction data sheet nor referred to Appendix C of the licensee's Q-List and Calvert Cliffs Instruction 228 for Safety classification and special maintenance requirements needed to maintain the environmental qualification of Class IE item, as required by Functional Test Instruction 104.

j Maintenance on the Steam Generator level transmitter 1 LT 1124C, which is required to operate in post-accident environment for the initiation of auxiliary feedwater systems, was not classified and conducted under the Qualification Maintenance Program requirements established in Calvert Cliffs Instruction 208.

Non-conformance Reports were not issued, in accordance with the requirements of Quality Assurance Procedure 17, when the Digital Volt Meters 8731, 10278 and 10273, used in post-maintenance test of Reactor Trip system, were out-of-calibration on January 8,1986, ,

April 14, 1986 and June 5, 1985 respectively. l 1

The test director for the Reactor Protection System Trip Breaker Surveillance did not check the position of the under voltage device prior to notifying the control room to close this breaker, as required by Surveillance Test Procedure (STP) M-200-1, Step II.B.2.

Appendix A 2 The recorded data for STP-M-511-1 and STP-0-65-2 were corrected -

without identifying the persons making the corrections and the dates on which the corrections were made, as required by Quality Assurance Procedure 7.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Baltimore Electric and Gas Company 1 is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time, l
