APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket Nos.

50-318

-Calvert Cliffs-Nuclear Power Station, Unit 2 License Nos. DPR-69 As a result of the inspection conducted on May 8-12, 1989 and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1989), the following violation was identified:

10 CFR 50 Appendix B, Criterion IX requires in part " Measures shall be established to assure the special processes, including welding, heat treating, and nondestructive testing, are controlled and accomplished by qualified personnel....."

The Calvert Cliffs Final Safety Analysis Report paragraph 18.9 Control of Special Processes requires in part " Nondestructive Examination personnel employed by or responsible to BG&E are certified according to applicable sections of the ASME Code and/or SNT-TC-1A and its Supplements. BG&E employees are trained and certified in accordance with a written procedure....."

BG&E Program for-Nondestructive Examination, NDE-CP-5.002, Revision 13, Qualification and Certification of Nondestructive Examination Personnel requires the following for BG&E employees:

6.3.3 All unlimited certified NDE examiners shall attend as a minimum the following annual training.

.6.10 Identification of Certified Personnel: Certified personnel shall be issued an identification card, which shall include photo,. certification level, expiration date, and signature of certifying Level III Examiner.

Contrary to the above, on May 12, 1989, the inspector identified that;

individuals for liquid penetrant testing; (2) no lesson plan record was consistently used to document NDE training, as a result the training was indeterminate; (3) certified personnel did not know when their certifications expired, as a result, at least in one case, the certifi-cation of a visual examiner had been expired for a period of seven months before decertification; and (4) the licensee's program failed to provide for certification of visual examination personnel for VT-1, VT-2, VT-3, and VT-4.

This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, the Baltimore Gas and Electric

, Company is hereby required to submit to this office within thirty days of the-date of the letter'which transmitted this Notice, a written statement or explanation.in reply, including: (1) the corrective steps which have been taken.and the' results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extend-ing this response time.
