APPENDIX A Notices of Violation Baltimore Gas and Electric Company Docket / License Nos.: 50-317; DPR-53 Calvert Cliffs Nuclear Power Plant 50-318; 0?R-69 As a result of the inspection conducted on April 1 - May 16,1988, violations of NRC requirements were identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Action," (10 CFR 2, Appendix C),

the violations are listed below:

A. Technical Specification 6.9.1.4 requires that all annual reports covering the activities of the unit, as described in Technical Specification 6.9.1.5.c, for the previous calendar year shall be submitted prior to March 1 of each year. Technical Specification 6.9.1.5.c requires that an annual report shall be made documenting all Oilures and challenges to the pressurizer power operated relief valves (PORVs) or safety valves.

Contrary to the above, as of April 1,1988, the licensee had neither pre-pared nor submitted the annual report for 1987 documenting all failures and challenges to the Units 1 and 2 pressurizer PORVs or safety valves.

This is a Severity Level V Violation (Supplement I) applicable to OPR 53 and 69.

B. Technical Specification 6.8.1 requires that written procedures be estab-lished, implemented, and maintained covering surveillance and test activ-ities of safety related equipment. Technical Specification 6.8.3 permits temporary changes to these procedures that do not alter their intent to be made provided that prior to implementation of the changes they are approved by two members of plant management staff, at least one of whom holds a Senior Reactor Operators license on the unit affected.

Contrary to the above, on April 4,1988, three temporary changes, that did not alter procedure intent, were made to Section II G of Surveillance Test Procedure M529-1 Revision 4, Containment Pressure Calibration, without the i approval of two members of the plant management staff prior to the imple- )

mentation of the changes, at least one of whom held a Senior Reactor Operators licensee on the affected unit.

This is a Severity Level V Violation (Supplement I) applicable to DPR-53.

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office within thirty days of the date of these Notices, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
