'As a result of the" inspection conducted on May 1-5, 1989 and in accordance

'with the " General Statement'of Policy and Procedure for NRC Enforcement ..

Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1989), the following

. violations were identified:

A. The Baltimore Gas and Electric Company Technical Specification, Section 6.8.1 states that written procedures shall'be established and implemented covering the applicable procedures recommended in Appeidix A:of Regulatory Guide 1.33, Revision 2.

Regulatory Guide 1.33, Revision 2, Appendix A, paragraph 9.a states in part, that: "The maintenance that can affect the performance of safety-related equipment should be properly planned and performed in accordance with written procedures, documented instructions...."

Contrary to the above, on May 5, 1989, the inspector determined there were.

no procedures for performing the following maintenance activities that can affect the performance of safety-related equipment:

1. No procedural requirements exist for SMB-00 and SMB-3 safety related MOV actuators to specify the frequency of inspections or the necessary criteria for stem lubrication of the actuators to assure proper.MOV operation. The inspector identified MOVs 2MOV653, 654 and 655 with unlubricated valve stems.
2. Criteria.were not 'specified to establish safety related MOV limit switch and bypass switch settings for proper MOV operation.

This is a Severity Level IV violation (Supplement I).

B. 10 CFR 50 Appendix B Criterion V requires that activities affecting quality be prescribed by procedures and shall be accomplished in accordance with these procedures.

i The Baltimore Gas and Electric Company Quality Assurance Policy, Revision i 19, Section 1B.5 requires that activities affecting quality shall be prescribed by procedures and shall be accomplished in accordance with these procedures.

Appendix A 2 The Baltimore Gas and Electric Set Point File Manual No. 14, Revision 4 requires that the torque switch settings for motor-orerated valve 2 MOV 653 be 1.5 for both opening and closing operations of the M0V.

Contrary to the above, on May 4,1989, the installed torque switch '

setting for motor-operated valve 2 MOV 653 was found non-conservatively set at 1.0 for both opening and closing of the MOV.

This is a. Severity Level IV Violation (Supplement I)

Pursuant to' the provisions of 10 CFR 2.201, v e Baltimore Gas and Electric Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3)'the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending this response time, j
