APPENDIX A NOTICE OF VILATION Baltimore Gas and Electric Company Docket / License Nos.: 50-317; DPR-53 Lusby, Maryland 50-318; DPR-69 As a result of the inspection conducted on August 9 - September 12, 1988, in accordance with the NRC Enforcement Policy (10 CFR 2 Appendix C) the following violation was identified:

Technical Specification 6.8.1 states that written procedures shall be established, maintained and implemented covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Rev. 2, February 1978.

Appendix A of Regulatory Guide 1.33, Rev. 2, 1978, requires procedures for .

performing maintenance where that maintenance can affect the performance of safety-related equipment. Maintenance Procedure HE 21, Rev. 2, Con-tainment Personnel Lock Interlock Bypass, specified that only one of two door interlocks be defeated to disable the interlock on the containment air lock.

Contrary to the above, on April 11, 1988, personnel performing HE 21 defeated two door interlocks for the purpose of disabling the Containment Air Lock Interlock.

This is a Severity Level IV Violation (Supplement I) applicable to OPR 53.

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to subnit to this office within thirty days of the date of this Nctice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violatie as; and (3) the date when full compliance will be achieved. Where good cat.se is shown, consideration will be given to extending the response time.
