I wish to make an amendment to Materials 1,1 cense number 45-23080-01, expiration date: June 30 1995, reference number iJ0-28653. The changes that 1 winh to make via this amendment are:

1) g be allowed to transport (approximately 2 miles) limited quantitieu of S (leas than 1 MCI) to the 1. IONS EYE BANK of Sentara Norfolk General llospital.

This material and any waste will be returned to Norfolk State University for subsequent analysis and disposal.

This amendment ir necessary to conduct research on f resh donated ocular tissue for the LIONS EYE IIANK. (see attached letter)

The Norfolk LIONS Medical Eye Bank and Research center la pleased to continue to supply your research group with ocular tissue (nontransplantable)  :

for biomedical research. As with previously-supplied tissue the Eye Bank '

will provide a brief medical history of each anonymous donor including:

age, sex, race, inanediate cause of death, post-mortem interval, and previous medical history. Although we can not predict the amount of tissue available for research each month, we are confident that there will he a minimum of 20 camples per month for use in your laboratory.
