NRC SPECIAL~ INSPECTION REPORT NO.. 999-90002/97-01 AND NOTICE OF VIOLATION
Dear Mr. Ruiz:
This refers to the inspection conducted on March 3-7, 1997, at your facility in Mayag0ez. Puerto Rico, and subsequent telephone conversations and reviews of records conducted through June 24, 1997. The enclosed report presents the results of the inspection and reviews. An exit interview for the onsite.
inspection was held with you and members of your staff on March 7. 1997.
During the inspection, records were reviewed and procedures were discussed with personnel. Based on the results of this inspection, one violation of NRC requirements was identified. The violation relates to the conduct of activities subject to licensing without a valid NRC license on one occasion between September 1993 and January 1994.
We recognize that the cause of the violation was lack of knowledge of NRC recuirements in combination with the confusion caused by Dr. V6zquez' sudden anc untimely death, and the desire to provide patient care after his death..
Based on the inspection, subsequent discussions with you and your staff and
, your March 21. 1997. misadministration report. 'the NRC has concluded that information regarding the violation. the corrective actions taken and planned to correct the violation and prevent recurrence, and the date when full compliance will be achieved have already been adequately addressed. These actions are outlined in the enclosed inspection report. Therefore, you are not required to respond to this letter unless the enclosed report does not accurately reflect your corrective actions or your position. In that case or if you choose to provide additional information, you should follow the .
Instructions specified in the enclosed Notice.
