PATIENT NOTIFICATION INFORMATION
Dear Dr. Feliciano:
N On April 28, 1998, the NRC issued a Notice of Violation and Proposed N
Imposition of Civil Penalty to Dr. Joss N. De Le6n (Enclosure 1).
'x As in the past, we understand that the Puerto Rico Department of Health may wish to issue a notice to each patient who received radiation overexposure as a result of pterygium treatments with a mis calibrated strontium 90 eye applicator by Dr. De Le6n. The notice would make patients aware of the misadministration and potential medical consequences and would also indicate that the patient should see a physician annually for followup. We understand that you are considering providing a similar notice to each patient annually E for as long as you deem necessary.
We have been informed that Dr. De Le6n attempted to notify all patients who received misadministration. Enclosure 2 consists of Patient Data Input Forms for the overexposed patients. Please note that Enclosure 2 contains medical file information that, under 10 CFR 2.790(a)(6), we do not disclose to the public. We would also appreciate you withholding this information from public disclosure. If additional information is needed, please do not hesitate to call me at (404) 562 4700.
