Initial Conditions: The following conditions exist for Unit 1:
The following information should be considered in an integrated, versus independent manner.
All conditions presently exist, and occurred 15 minutes ago.
-The plant is at 100% power.
--The ARO position for Shutdown and Control bank rods is 228 steps. The RO has just completed quarterly control rod exercising per OSP-14410-1, and reports that rod H8 on CBD indicated 228 steps, when the rest of the bank indicated 222 steps by DRPI and 218 steps by demand.1 & C was called to investigate and does not report any abnormal indications on the rod control system.
-Containment Sump level indicator 1LT-7789 is failed high
-A train MDAFW pump surveillance has just been com, 3d, and the BOP reports that it failed to meet the acceptance criteria of OSP-14545-1.
-NSCW pump 6 is out of service for motor rewinding, when 1HV 11613, the pump discharge valve for NSCW pump 4 spuriously closes and the MOV burns up causing a small fire in the motor housing. The fire is quickly (less than 10 minutes) put out using a fire extinguisher.
The operators shutdown Train 8 of NSCW per the AOP. Maintenante reports that it will be at least 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> before the valve can be replaced or reoaired.
-Chemistry reports that they have determined that #3 SG is indicating a leakrate of 0.55 gallon per minute.

Dets Approved Page Number 8/5/97 CONTROL ROD OPERABILITY TEST 1of7 1.O PURPOSE 1.1 The purpose of this procedure is to demonstrate the operability of the Shutdown and Control Rods.
1.2 This P.est satisfies surveillance requirements..of Technical Specification SR 3.1.4.2.
1.3 The frequency of this surveillance is at least once every 92 days.
2.0 APPLICABILITY
~
This surveillance is required for Modes 1 and 2.
3.0 PRECAUTIONS AND LIMITATIONS 3.1 Changes.in turbine load and boron concentration should be avoided during this test.
3.2 Monitor reactor during all rod manipulations for any abnormal conditions.
3.3 Ensure that plant conditions are stable after a bank has been tested before testing the next bank.
3.4 Before transferring to AUTOMATIC reactor control, ensure Tavg is within 11*F of Tref.
3.5 ENSURE that both groups for each rod bank are at the same step counter position prior to repositionirg bank.
3.6 Overlap rod bank motion is preserved only if the Rod Bank Selector Switch is in MANUAL or AUTO.
3.7 Maintain Tavg/ Tref deviation less than or equal to 3*F.
3.8 Notify Unit Shift Supervisor (USS) immediately if ut: satisfactory rod movement occurs.

The following steps apply to each shutdown and control bank to be tested.

This test is applicable to each shutdown and control bank not fully inserted.

A reactor startup or shutdown, moving rods at least 10 steps, will satisfy this surveillance. The following instructions are written for the normal, all banks withdrawn, condition.
5.1 RECORD the INITIAL Group Step Counter and Individual Rod Position Indicator readings for the bank being tested on Data Sheet 1. Additionally, if moving a control bank, record the initial P/A converter reading on data sheet 1.
5.2 PLACE ROD BANK SELECTOR SW 1-HS-40041 to the bank being tested.
5.3 Using ROD MOTION SWITCH 1-HS-40040, INSERT the bank being tested at least 10 steps as indicated on group step counters.
VERIFY RODS IN light illuminated and OBSERVE a change in position for each rod on the DRPI Display Panel.

5.8 If required, RESET the bank being tested to the new ALL RODS OUT position per 13502-1, " Control Rod Drive And Position Indication System".
5.9 At the completion of all bank testing, ROD BANK SELECTOR SW 1-HS-40041, should be placed in AUTO or left in MANUAL as directed by the USS; IV required.

- Aux. Feedwater 1 HV 5139 Valve is to be repacked, PM, VOTES test, and have the grease replaced. Manual valve 1 1302 U4 045 has been CAUTION tagged shut per MOV clearance to prevent gravity feed to SG # 1. No other valve work is in progress on Train A.
1 HV 1975 has been CAUTION tagged shut to maintain Containment integrity. No work is in progress on ACCW.
- CVCS Seal Return 1 HV 8100 Valve has been disassembled to rework the seat.1 HV 8112 has been CAUTION tagged shut to maintain Containment integrity. No other valve work is in progress on CVCS Seal Water Return.
- CVCS Charging 1-HV-8106 Valve is to have springpak tested and a V0TES test performed.1 HV 8105 is red tagged shut under the CVCS clearance and is identified on the clearance as maintaining Containment integrity. No other valve work is currently in progress on CVCS Charging.
- Cnmt Spray 1 HV 9001B Valve is to have a VOTES test performed. No other valve work is in progress on Train B Containment Spray.

l This procedure prescribes the method to record the failure to meet the Limiting Conditions for Operation (LCO), or Technical Requirement, the associated ACTION requirements, any change in status effecting the ACTION, and the return to compliance with LCO/TR.
This procedure also implements the Safety Function Determination Program and therefore ensures that the proper actions are taken upon a failure,to concurrently meet two or more Technical Specifications LCO's such that multiple inoperabilities of Structures, Systems, or Components (SSC) do not result in an undetected LOSS OF SAFETY FUNCTION. This procedure also ensures that the allowed out of service time of SUPPORTED SYSTEMS is not inappropriately extended as a result of multiple SUPPORT SYSTEM inoperabilities.
2.0 DEFINITIONS 2.1 LIMITING CONDITION FOR OPERATION (LCO)
A condition specified in the plant Technical Specifications (TS) which limits unit operations.
An LCO may be entered by an equipment malfunction or a change in a unit parameter.
In addition, an LCO may be used when the requirements of the Offsite Dose calculation Manual (ODCM) Sections 1.5 and 2.5 are not met to administratively track compliance with the ODCM l
program.
If an LCO is not met, the associated ACTION requirements shall be met.
2.2 TECHNICAL REQUIREMENT (TR) l A condition specified in the plant Technical Requirements Manual (TRM) which limits unit operations. A TR may be entered by an equipment malfunction or a change in a unit parameter. If a TR is not met, the associated ACTION requirements shall be met.

Specifications. However, TRs and TRSs are treated as plant

A method of tracking an equipment malfunction or change in plant parameter which would restrict unit operation in another mode or prevent a mode change in which it would become applicable, or may become an LCO/TR for the present mode should other Technical Specification related equipment or redundant safety related equipment become inoperable.
In addition, Information only LCO/TR's may be applicable to requirements found in the ODCM Sections 1.5 and 2.5.
During a refueling outage, Information Only LCOs should not be prepared for conditions that are not applicable in the present operating mode unless used for tracking for entry into a mode in which a transition is to be directly made.
2.4 BASE LCO The SUPPORT or SUPPORTED SYSTEM LCO in which the initial inoperability solely resides. This may result in multiple LCOs if the inoperability is caused by support systems outside of Technical Specifications.
2.5 LOSS OF SAFETY FUNCTION (LOSF)
A LOSF exists when, assuming no concurrent single failure, a safety function assumed in the accident analysis cannot be performed.
2.6 SUPPORTED SYSTEM A SSC, required by the Technical Specifications, which requires a SUPPORT SYSTEM to ensure its safety function can be performed.
Process parameters or operating limits are not a SUPPORTED SYSTEM for the purposes of implementing LCO 3.0.6.
2.7 SUPPORT SYSTEM A SSC which is needed by another Technical Specification LCO required SSC to perform a safety function.
Example: The Component Cooling Water System (SUPPORT SYSTEM) is required by the Residual Heat Removal System (SUPPORTED SYSTEM) to fulfill its safety function. A SUPPORT SYSTEM may also be a SUPPORTED SYSTEM. Example: The Component Cooling Water System requires the Nuclear Service Cooling Water System to fulfill its safety function.

Example: The Digital Rod Position b.y Indicators (DRPI) are used to monitor control rod insertion limits, however; inoperability of DRPI does not result in the control rods not being within insertion limits. Control rod insertion limits are monitored separately and actions taken as appropria.te when insertion limits are not met or Surveillance Requireme nts not performed when required.

Example: Exceeding control rod insertion limits could affect hot channel factors.
3.0 PROCEDURE NOTE During a refueling outage, Information Only LCO/TR's should be prepared for conditions that are applicable only in modes into which entry is to be directly made.
1 3.1 LCO/TR STATUS SHEET PREPARATION When a unit fails to meet an applicable LCO/TR and requires initiation of an ACTION statement, or equipment malfunction requires an Info LCO/TR, the Unit Shift Supervisor (USS) shall complete Figure 1, "LCO/TR Status Sheet"Section I, inform the Shift Superintendent (SS), and log'the status sheet on Figure 2, "LCO/TR Status Log" prior to completing his shift.

Completion of an LCO/TR Status Sheet is not required for the performance of surveillances or short term entry into an LCO/TR that does not continue past a shift turnover if the USS logs the following items in the USS Log:

3.2 CONVERSION OF LCO/TR(s) TO IMFO LCO/TR(s)
When the unit enters a mode where a given Technical Specification is no longer applicable, LCO/TRs against that specification should be converted to Info LCO/TRs.

3.3 CONVERSION OF INFO LCO/TR(s) TO LCO/TR(s)
An Info LCO/TR should be closed out and a new LCO/TR status sheet generated.
3.4 LCO/TR STATUS BINDER An LCO/TR Status Binuer shall be maintained for each unit by the SS.
The binder is a looseleaf notebook containing 3 sections:
3.4.1 Part I.
LCO/TR Status Log.
The first part shall list, on Figure 2, "LCO/TR Status Log", all LCO/TR's in their order of initiation.
New log pages should be placed in the front, so that opening the front cover will expose the most recent entries.
The LCO/TR Status Log Sheets will be retained in the binder until all listed LCO/TR Status Sheets have been retired, then the log sheet should be removed and sent to Document Control.
A copy of the complete status log sheet should be placed in Part III.
3.4.2 Part II.
Active LCO Status Sheets.
Part II contains the original of Figure 1, "LCO/TR Status Sheet" forms for all LCO/TR's that are in effect.
New entries are placed in the front.
LCO/TR and Info LCO/TR forms may be kept separately in Part II if desired.
3.4.3 Part III.
Completed LCO/TR Status Sheets.
Part III contains copies of LCO/TR Status Sheets for LCO/TR's that have been restored.
Sheets are filed in order of their LCO/TR number.
Copies of completed sheets should be retained in the binder for at least 30 days after they have been closed out.

LCO 3.0.6 provides an exception to LCO 3.0.2 for SUPPORTED SYSTEMS by not requiring the Required Actions for the SUPPORTED SYSTEMS to be performed when the failure to meet an LCO is solely due to the inoperabilit" of a SUPPORT SYSTEM.
In this situation, LCO 3.0.6 requires o-the Required Actions of the SUPPORT SYSTEM to be performed.
ace " cascading" is not required in this case, a possibility exists that unrelated concurrent failures of more than one SUPPORT SYSTEM could result in the complete loss of both trains of a SUPPORTED SYSTEM.
Therefore, upon a failure to meet two or more LCOs during the j
same time period, an evaluation shall be conducted to determine if a LOSS OF SAFETY FUNCTION (LOS F) exists. This LOSF Evaluation satisfies the criteria of Technical Specification Administrative Control 5.5.15, Safety Function Determination Program.
NOTE If the failure of a Technical j
Specification required SUPPORT SYSTEM results in the inoperability of a system outside of the Technical Specifications, and that system is subsequently relied upon by a SUPPORTED SYSTEM to remain OPERABLE, then LCO 3.0.6 would apply and only the SUPPORT SYSTEM's Required Actions would be entered. Example: If 1NB10 is de-energized, the operability of D/G
'1B' or the Pressurizer Heaters may be impacted.
3.5.2 A single component inoperability can result in multiple inoperabilities within a single train and affect multiple Technical Specification LCO's. LCO 3.0.6 limits the amount of
" cascading" of actions that is required when an inoperable SSC renders a SUPPORT SYSTEM inoperable.
When a SUPPORT SYSTEM or SUPPORTED SYSTEM is rendered inoperable, it is declared inoperable. The intent of LCO 3.0.6 is not to preclude declaring a SUPPORT or SUPPORTED SYSTEM inoperable. LCO 3.0.6 merely suspends performance of the SUPPORT or SUPPORTED SYSTEM's " Required Actions".
A Loss of Safety Function evaluation must be performed for each inoperability of a SUPPORT or SUPPORTED SYSTEM (s).

3.5.3 The LOSF Evaluation must be reinitiated whenever additional equipment is declared inoperable. This includes LCOs with Required Actions that specify declaring a component inoperable after a specified Completion Time has expired.
3.5.4 Determine if the inoperable SSC is directly addressed by a Technical Specification LCO. If the SSC is addressed in Technical Specifications, proceed to step 3.5.6. Otherwise, continue with step 3.5.5.
3.5.5 If the inoperable SSC is not addressed in Technical Specifications, determine if it renders another SSC directly addressed in Technical Specifications inoperable. If the initially inoperable SSC does not render a SSC addressed in Technical Specifications inoperable, no further action is required, and no further LOSF evaluation is required.
3.5.6 Enter the applicable Technical Specification conditions and required actions for the inoperable SSC determined in Step 3.5.4 or 3.5.5 above.
3.5.7 Determine if the unit has entered the required actions of more than one Technical Specification LCO. If only one LCO action has
.been entered, follow the Required Action and no LOSF exists, and no further evaluation is' required.
3.5.8 If more than one LCO's Required Actions have been entered, the.'
determine if all the LCO's have been entered for the same train.
If the LCO's have all been entered for the same power train, then no LOSF exists, and no further evaluation is required.

3.5.10.2 If a LOSF is determined to exist, the appropriate Conditions and Required Actions of the LCO in which the LOFF exists shall be entered. If no Condition within the LCO addresses the LOSF, then LCO 3.0.3 shall be entered.

Assigned Task: It is estimated that it will take the operator 2 minutes each way to get to and from the valve once he has entered AB-15. The calculated average area dose rate that the operator will travel through to and from the valve will be 3750 mr/hr. Using the provided survey map determine how long the volunteer operator may actually spend at 1HV-8821 A throttling the valve to the desired flowrate if he stands approximately 1 foot from the valve. The operator is authorized to receive up to the maximum allowed emergency dose for performing this evolution for saving important plant equipment.
