During the NRC inspection conducted on October 5.1997 through November 15.
1997, a deviation was identified from commitments you made as part of
knendment 132 to the Unit 2 Technical Specification.
In accordance with the
" General Statement of Policy and Procedure for the NRC Enforcement Actions."
NUREG - 1600. the deviation is listed below:
In the application for Technical Specification Amendment 132 for Unit 2
dated October 1. 1993, revised January 6. 1994, and supplemented February
3. 1994, a commitment was made to treat an alternate leakage path from
t
the main steam isolation valves to the main condenser through a main
steam isolation valve drain line and valve 2B21F021 as American Society
of Mechanical Engineers (ASME)Section XI Class 2 for repairs / replacement
under section XI.
The commitment was to be implemented prior to resi. art
from the Spring 1994 Unit 2 refueling outage.
Contrary to the above. Unit 2 was restarted on April 28. 1994. following
the Spring refueling outage with re) airs / replacement work perforrred under
Design Change Request 96 606, for tle replacement of valve 2B21F021 and
approximately four inches of piping in an alternate leakage path from the
main steam isolation valves to the main condenser, which were not +reated
as ASME Section XI Class 2.
Please provide to the U.S. Nuclear Regulatory Commission. ATTN: Document
Control Desk. Washington. DC 20555, with a copy to the Regional Administrator.
Region II, and a copy to the NRC Resident inspector at the Hatch facility, in
writing within 30 days of the date of this Notice. (1) the reason for the
deviation, or, if contested. the basis for disputing the deviation. (2) the
corrective steps that have been taken and the results achieved. (3) the
corrective steps that will be taken to avoid further deviations, and (4) the
date when your corrective actions will be completed. Where good cause is
shown, consideration will be given to extending the response time.
